Form relates to the aesthetic aspect of a website. There are some websites that look pretty spectacular and it is obvious a great deal of work has gone into the visual design. Perhaps it makes good use of colour, photographs, and flash animations to really catch the eye of the viewer.

Function relates to how well the website integrates with components outside of itself. For example, a site that reaches into an order entry database to display information to a customer about their account would be rated as highly functional.
